Installation of BraunAbility side-entry, lowered floor, powered in-floor ramp for 2021 or newer Toyota Sienna. Provider must be enrolled with Medicaid as a PVM waiver service provider to be eligible to deliver this service but all qualified adaptive vehicle providers are welcome to bid. Existing bidders do not need to re-submit a quote.
